About This Framework

This framework was developed from production multi-agent operations where autonomous AI agents performed real engineering work under delegated authority. Every governance mechanism was created in response to a documented failure. The methodology is empirical, not theoretical.

The theoretical foundations draw from established literature in safety engineering, military operations, and organizational reliability: Two-Person Integrity (U.S. DoD, 1962), the Swiss Cheese Model (Reason, 1990), Drift Into Failure (Dekker, 2011), Normalization of Deviance (Vaughan, 1996), the OODA Loop (Boyd, 1976), and High-Reliability Organizations (Weick & Sutcliffe, 2007).
